There are some system requirements for using the PDF or MS Excel online application forms.
For the Adobe PDF format Registration Form:
- If you are using Internet Explorer, the minimum version that will work with the site is Version 5.0 With Service Pack 2. If you are using Netscape, the minimum version that will work is Version 6.2.
- You will need Adobe Acrobat Reader in order to download and fill the Adobe PDF format Registration Form. The minimum version that will work with the site is Version 5.0. If you do not have Adobe Acrobat Reader V5.0 or higher, you may download a free copy from Adobe’s website.
- You will be sending the completed Registration Form back to us via email. Selecting Submit at the bottom of the form will open your email client. Our email addresses will be automatically filled in and you will see a data file attached. You do not have to enter anything into the body of the letter, just select Send. We will receive the data file and it will be repopulated on the same form in our computers. Note: you cannot save this form or the data. Once you select Submit, the data is removed from your computer. You can print a copy, and we recommend you do this as a backup. If there are any problems, the printed copy can be scanned and sent to us as an email attachment for manual entry.
- Depending on the speed of your computer, it may take several minutes to download and complete the form.
For the MS Excel format Registration Form:
- You will need MS Excel. Most versions should work.
- The form can be filled out on your screen, saved and sent as an email attachment. Some fields do not require an entry. They are automatically calculated for you.
- It is a good idea to print a copy of the completed form as well as saving it to your hard drive.
